Purpose: The goal of the Queens HIV+, Hepatitis C and Minority Re-Entry Project is to provide HIV+ and Hepatitis C prevention and intervention services to the criminal justice population in Queens. This population includes individuals returning to Queens from prison. This project is in collaboration with several treatment agencies and Elmhurst Hospital.
Program Description: Individuals involved in the criminal justice system are also significantly more likely than the general population to be (or be at risk for) HIV+ or Hepatitis C- infected. As a result, special attention needs to be paid to the ongoing medical and psychological treatment needs of this population.
Clients in EAC’s Queens TASC Program, or individuals referred from Parole, meet with a case manager who assesses their level of functioning, identifying needs and making referrals for necessary services. Clients may participate in HIV or Hepatitis C services offered at Elmhurst Hospital’s program. EAC case managers work with the client and family to develop long term care plans which can address the unique needs of these clients, including appropriate nutritional planning, medical support, housing, public assistance benefits and treatment resources. AIDS testing and counseling are also available. Program staff monitor treatment and report to the Court as needed.
Funding: Program funding is provided by the U. S. Department of Health and Human Services Substance Abuse and Mental Health Services Administration.
